- After accessing its official site page, you will first have to get login access by entering your specific medical record number, user ID & password in the respective fields to get logged into an account.
- If you are a new visitor on this site, you have to create your new account by clicking on an option “Create a new account” for completion of the further process.
- Next, you have to follow the procedure for creating a new account such as activation code, medical record, and date of birth and in the end, click on the “submit” button to finalize the process.
